当院は、外国人向けにPET/CT(FDG)検査によるがん検診及び頭部MRI検査による脳ドックを提供いたします。
日本に長期在留されている方や短期訪問の方も、検診を受けることができます。

検診メニュー・料金

※1 読影レポートは、原則日本語です。
※2 検査結果は、日本国内で受け取れる場所に郵送いたします。
　　申込時に、住所の記載をお願いいたします。
　　直接来院して受け取ることも可能ですので、お申し出ください。
※3 医師による結果説明を希望される場合は、ご相談ください。
　　別途外来受診の費用が発生いたします。
※4 当日、自己都合で検査を中止される場合は、検診料金を全額請求いたします。
※5 日本語の読み書きが困難な方は、日本語ができる方の同伴をお願いいたします。
　　同伴者が見つからない場合は、同院で通訳サービス（有料）を手配することが
　　可能です。
　　通訳者の人数には限りがありますので、必ず事前にご相談ください。

お申込みいただいた内容に基づいて、検診の受入れ可否を判断いたします。
受入れ可能な場合、以下、書類をメールで送信いたします。

✔　PET-CT（FDG）検査同伴に関する説明と同意書
✔　MRI検査を受けられる方へ

当院：検査日の予約確定、説明用紙送付

検査日の予約が完了しましたら、検査の説明用紙を送付します。
説明用紙には、検査予約票、当日の流れなどが記載されています。
事前にご一読いただきますようお願いします。

検診者側：説明用紙をお持ちの上、検査日当日来院

当日予約票に記載された禁食時間や受付時間を確認した上、ご来院ください。

キャンセルについて

キャンセルの連絡方法

申込みをキャンセルする場合は、検査前日（日曜日・日本の祝日の場合は、前々日など休日以外）の15時までに下記へご連絡ください。

キャンセル料について

・検査前日（日祝日の場合は、検査前々日）の15時まで：無料
・検査前日（日祝日の場合は、検査前々日）の15時以降：税込49,720円（FDG薬剤費用として）
検査当日に連絡なしで来院されなかった場合も同様に上記のキャンセル料金を請求いたします。
キャンセル料の請求時、ご本人による支払いが確認できない場合は、申込書記載の連帯保証人に請求させていただきます。ご了承ください。

検査機器の故障や薬剤輸送中の事故などトラブルについて

Our hospital offers cancer screenings using PET/CT (FDG) and brain checkups using head MRI scans for foreign residents and visitors to Japan. Both Long-term residents and short-term visitors in Japan are welcome to undergo these screening.

PET/CT (FDG) Cancer Scan Course and Fee

※1 The test results report will be provided in Japanese.
※2 The test results will be mailed to an address within Japan. Please provide the address at the time of application. If you prefer, the results can also be given in person at our hospital. Please inform us in advance.
※3 If you would like a doctor to explain the results, please contact us. Additional outpatient fees apply.
※4 If you cancel the examination on the day for personal reasons, the full examination fee will be charged.
※5 If you have difficulty reading or writing Japanese, please bring someone who can speak Japanese with you. If this is not, the hospital can arrange an interpreter for a fee. As the number of interpreters is limited, please be sure to consult with us in advance.

　Based on the information provided, we will determine whether the examination can be accepted. If it can be accepted, we will send the necessary documents listed below by email.

Hospital Side: Sending of Information and Appointment Form

　After confirmation of your appointment, we will send you a PET-CT (FDG) exam instruction sheet. The instruction sheet contains details such as your exam reservation slip and the schedule for exam day. Please be sure to read it in advance.

Applicant Side: On the Day of the Examination

　On the day of the examination, please check the fasting time and check-in time on your reservation slip before arriving at our hospital.

About Cancellation

How to Notify of a cancellation

　If you need to cancel your appointment, please contact us by 3:00 pm on the day before your examination (or two days before if the day before is a Sunday or a holiday) at the number below:

Cancellation Fees

・By 3:00 pm on the day before your examination (or two days before if your examination falls on a Sunday or a holiday)

・After 3:00 pm on the day before your examination (or two days before if your examination falls on a Sunday or a holiday)
　　　: ¥49,720 (including tax, for FDG drug costs)

　If you do not show up on the day of your examination without prior notice, the same cancellation fee will be charged. If we are unable to confirm payment from you, we will charge the guarantor listed on your application form. Thank you for your understanding.

本院为外国人提供癌症筛查体检及脑部精检。
癌症筛查体检及脑部精检，分别进行PET/CT(FDG)检查及头颅磁共振检查。
无论长期居住在日本或短期访问，都可以接受该体检。

体检内容·费用

※1 原则上只提供日文的读影报告书。
※2 检查结果将邮寄到日本国内可以接收的地址。请在申请体检时，填好地址。也可以直接来院收取，请您提前告知院方。
※3 如果您希望改天由医生说明检查结果，请事先咨询相关事宜。医生说明的门诊费用另算，请您知晓。
※4 检查当天，来院之后由于自身原因要求停止检查时，也会发生全额体检费用，望知晓。
※5 日语读写困难者，需要会日语的人陪同做检查。若您找不到陪同做口译的人，请提前咨询我们。本院也可以提供体检口译服务（有偿），口译人员人数有限，请务必提前联系。

体检申请人：体检当天，请您持上述说明书来院。

请您提前确认预约票的禁食时间及接收时间，在时间内来院。

关于取消

取消时的联系方法

申请完毕之后，如果您希望取消该体检，请在 检查日期前一天（周六，日本公休日的情况下，请在检查2天前）15点之前，联系以下窗口。

关于取消费用

• ・检查前一天（周日，日本公休日的情况下，检查前2天）的15点为止取消：不发生费用
• ・检查前一天（周日，日本公休日的情况下，检查前2天）的15点之后取消：将发生49，720日元的取消费用（含税）。该费用是FDG药剂费用。
   

检查当天，无故取消不来院的情况，也将发生上述取消费用。
本院将向本人请款，如果本人没有支付，担保人也将发生连带支付责任。望知晓。
